Student Min Winter Retreat Scholarship Application

Scholarship funds are made available for students to attend various activities and events such as camps and retreats. Some (but not all) of the factors used to determine need are the amount in the Scholarship Fund at any given time, the number of students requesting scholarship assistance, the need of the applicant, multiple family members desiring to attend, etc. In order to make informed decisions regarding scholarship funds, we ask that you complete and submit this form as soon as possible.

*There are many ways to pay this blessing forward. How could you and/or your student do that? 
If you are awarded a scholarship and withdraw from the trip without medical cause within two weeks of departure, you may be required to pay all or a portion of the money back to the scholarship fund. Many people give sacrificially to the Scholarship Fund and since funds are paid out to event venues in advance, the scholarship money may be lost when another student could have used the funds. 
I understand that if my student withdraws without medical cause within two weeks of departure that I may be required to pay back all or a portion of the advanced funds.
